Abstract

360° CHAT-TMD RP2 ABSTRACT Temporomandibular disorders (TMD) affect 10% of the population, causing significant pain and functional impairment. Current diagnostic and treatment practices are often inconsistent due to the subjective nature of symptoms and comorbidities like chronic overlapping pain conditions (COPCs). The overarching goal of the proposed U54 Program, 360° Collaborative Hub for AI in TMD Research (360° Chat-TMD), is to develop a scientifically robust, interdisciplinary approach to TMD management, clinical care, and translational research utilizing advanced AI/ML techniques. By integrating perspectives from individuals, healthcare systems, and communities, we aim to revolutionize TMD research and care. Supported by a multidisciplinary team from the University of Michigan, Harvard University, Mass General Brigham, and patient communities, this project leverages extensive expertise in TMD/pain research and groundbreaking technologies. The 360° Chat-TMD Program aims to generate mechanistic, clinical, and outreach predictive models and develop functioning AI/ML tools for personalized phenotyping and to establish scalable clinical standards for TMD care. The three primary Program aims include: (1) Whole-Person Research to enable precision medicine through AI/ML models integrating clinical, (neuro)imaging, and phenotyping data across diverse TMD subtypes, focusing on molecular and central nervous system changes; (2) Whole-Healthcare Research to classify TMD subtypes, predict disease progression, and optimize treatments using AI/ML models that incorporate pain widespreadness, COPCs, and imaging data to enhance clinical decision support and treatment personalization; and (3) Whole-Community Research and Collaboration using a mobile chatbot for remote outreach, phenotyping, and patient education, integrating remote BODY-AI indexed maps with an active chatbot tool supporting mechanistic models of TMD and COPC, cohort classification, and treatment prediction with direct community engagement. Utilizing advanced AI/ML technologies and comprehensive patient data integration, this Program aims to transform TMD research, enhance clinical practice, and build sustainable community engagement. The Program will deliver practical tools for clinicians, researchers, patients and policy-makers, as well as promote advancing precision dental medicine striving to improve TMD healthcare outcomes.
