Description
Notice of Intent to Sole Source: HT941026N0114 THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division - West (HCD-W), Alpha Branch intends to negotiate and award a firm fixed price contract for Pentacam Equipment Repair and Maintenance Services for OCULUS INC. at Naval Hospital Camp Pendleton in accordance with Federal Acquisition Regulation (FAR) 6.302-1 Sole Source acquisitions and FAR 12.102(a), only one source being available to meet the Government�s needs to: OCULUS INC. 17721 59TH AVE NE ARLINGTON WA 98223-6446 UEI: H6RTL2ZAKEH1 CAGE: 8AJX5 The North American Industry Classification System (NAICS) code for this requirement is � 811210: Electronic and Precision Equipment Repair and Maintenance, the business size standard is not to exceed $34M (Dollar amount). The Product Service Code (PSC) is J065: Maintenance, Repair, and Rebuilding of Equipment- Medical, Dental, and Veterinary Equipment and Supplies. This requirement is a repair and maintenance agreement for government-owned Pentacam Equipment at Naval Hospital Camp Pendleton. The proposed contractor is OCULUS INC. Location: OPHTHALMOLOGY DEPARTMENT, BLDG H135, NMRTC, 200 Mercy Circle, Camp Pendleton, CA 92055 Estimated Period of Performance (PoP): BASE YR POP: 10/01/2026 to 09/30/2027 OPTION YR 1 POP: 10/01/2027 to 09/30/2028 OPTION YR 2 POP: 10/01/2028 to 09/30/2029 Based on market research and previous acquisition history, it has been determined that OCULUS INC. is the sole provider of service for government-owned Pentacam Equipment. This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required Pentacam Equipment Repair and Maintenance Services. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
