Notice of Intent to Sole Source HT941026N0125 THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE PURCHASE ORDER CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Healthcare Contracting Division - West (HCD-W), Alpha Branch intends to negotiate and award a firm fixed price purchase order (PO) contract for service maintenance for COX COMMUNICATIONS HAMPTON ROADS, L.L.C. at Naval Hospital Camp Pendleton in accordance with Federal Acquisition Regulation (FAR) 6.302-1 Sole Source acquisitions and FAR 12.102(a), only one source being available to meet the Government�s needs to: COX COMMUNICATIONS HAMPTON ROADS, L.L.C. 1341 CROSSWAYS BLVD STE 100 CHESAPEAKE, VA 23320 USA The North American Industry Classification System (NAICS) code for this requirement is �541519 Other Computer Related Services, the business size standard is $34M (Dollar amount). The Product Service Code (PSC) is DD01 � IT AND TELECOM - SERVICE DELIVERY SUPPORT SERVICES: ITSM, OPERATIONS CENTER, PROJECT/PM. This requirement is a service and maintenance agreement for Cable Service at Naval Hospital Camp Pendleton. The proposed contractor is COX COMMUNICATIONS HAMPTON ROADS, L.L.C. Location: Primary location - BLDG H200 200 Mercy Circle, Camp Pendleton, CA 92055. See SOW for additional locations. Period of Performance (PoP): BASE YR POP: 10/1/2026 to 9/30/2027 OPTION YR 1 POP: 10/1/2027 to 9/30/2028 OPTION YR 2 POP: 10/1/2028 to 9/30/2029 OPTION YR 3 POP: 10/1/2029 to 9/30/2030 OPTION YR 4 POP: 10/1/2030 to 9/30/2031 -8 Extension: 10/1/2031 to 3/31/2032 Based on market research and previous acquisition history, it has been determined that COX COMMUNICATIONS HAMPTON ROADS, L.L.C. is the sole provider of service for cable This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required Medbridge Essentials software. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
